Both the Reablement Service ( community based ) and Intermediate Care ( bed-based ) support Rotherham residents to meet their own goals and realise their independent potential. The purpose of each service is to help people who have experienced deterioration in their health, and/or have increased support needs, to relearn skills or develop confidence in managing their daily needs so that they are able to remain in their own home wherever possible and safe.
Within each service area we have Reablement Workers who deliver direct care and support to people and promote a range of supportive options including Assistive Technology and close working with other agencies in managing overall need. Our focus is to support people and their carers to build on their strengths, assets and abilities to maximise their independence and wherever possible self-manage.
The team receives their work from each of the above services, and works closely with health professionals, most commonly Occupational Therapists and Physiotherapists, in supporting our customers to increase their strengths and abilities. Through weekly multi-disciplinary meetings, we support each other in taking a positive approach to risk and identifying holistic solutions to identified needs. This integrated working promotes more positive engagement with our customers and their representatives, enabling them to directly feel the benefits of a ‘one team’ approach.
Assessment and Review Co-ordinators will need to have a good working knowledge of Adult Social Care and the legislative framework that informs our work: for example, the Care Act and Mental Capacity Act. Experience in completing assessments within this legislation, as well as competence at managing a case load and any associated risks is essential. Everyone responds to Adult Safeguarding concerns if they arise, and the Assessment and Review Co-ordinator will need to fully understand the principles of ‘making safeguarding personal’ in any such activity.
