We are recruiting 1 Team Around the Person (TAP) Coordinator vacancies. You will be part of a highly committed and supportive team, working to facilitate multi-agency support coordination for Individuals in Sheffield.  We are person-centred and passionate about achieving outcomes for those Individuals in vulnerable situations. 

We are looking for candidates that have experience in providing support or advocating for individuals, are accountable for ensuring the highest professional standards and high level of conduct whilst ensuring good practice.

You will need to be innovative, solution focused, and person centred, and have excellent customer service skills, be a confident communicator, excel in building professional multi-agency relationships and have a sensitive approach to dealing with often complex situations.

The role will be to support multi-agency working, to help co-ordinate support for individuals within Sheffield to create a joint tailored action plan that considers ‘what the individual needs and wants. This role will involve working across partnerships and agencies in Sheffield to make sure individuals get the right support at the right time.

You'll need to have significant experience undertaking complex investigations, completing risk assessments, presenting evidence, and chairing Multi-Disciplinary Meetings (MDTs) with a range of partners including health.

We particularly welcome candidates with support and healthcare experience. 

This a temporary fixed contract until March 2025

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
