Our Reablement Service are seeking to appoint a Practitioner, working a 26.25 per week permanent contract. This essential service works 7 days per week, 52 weeks a year, providing the best possible quality of care in the community.

The successful person will be a car driver and be prepared to work a 4-week rota during the hours of 07:00am – 15:00pm and 14:30pm-23:00pm.
This exciting role will lead a team to deliver high quality care and support to the most vulnerable people of our district, meeting their needs, and promoting person centred care at every opportunity.
This post is subject to the Fluency Duty under Part 7 of the Immigration Act 2016. The post holder is required to demonstrate a proficient level of fluency under the Duty as detailed within the job specification for the post”

The aim of our service is to enable service users to remain in their own homes by promoting independence and delivering a quality service.
To ensure individuals are treat with dignity and respect and to work alongside other professionals and agencies in a person-centred way to deliver effective services by working within national guidelines and standards.
The Reablement Service provides short term care, usually delivered in the person’s own home and is offered to people with disabilities and those who are recovering from an illness or injury and have the potential for significant improvement.
The purpose of Reablement is to help people who have experienced deterioration in their health or have increased support needs, to relearn skills required to keep them safe and independent at home.
We provide a bridging service which provides care and support to people who are waiting for a long-term package of care to be agreed or to bridge a period of time before a package of care can restart.
Our resettlement service provides care and support for a short period of time to settle people back into their previous daily routine and gives extra support with confidence building and time to readjust after a period of time away from home.
We also provide emergency care and support response to people who need extra support whilst further assessments can be completed.
The successful candidate will be given a comprehensive induction programme, full training to deliver high standards of care, and we will fund the qualification for you on successful completion of the probation period.
With a salary of £27,711 to £30,559 (FTE), enhancements for weekend and bank holiday working, plus overtime opportunities and 29 days holiday (FTE), why wouldn’t you want to choose a career in care?
