Are you a dedicated and ambitious Qualified Social Worker seeking a rewarding role?

We are looking to recruit a social worker who is enthusiastic, compassionate, positive thinking and self-motivated. The individual must display a desire to support people to maximise their independence. The vision is that the Social Worker would develop constructive working relationships with community / health colleagues, and voluntary sector partners. You will take the lead in promoting strengths based practice to develop new ways of working and ensure integrated and targeted case management.

We are looking for an experienced and registered social worker who is keen to support people in our local area, and carry a varied caseload.  In this role, you will be working with a vast client group. The role involves undertaking case management duties, under the advice and guidance of the Team Manager / Assistant Team Manager and carrying out the statutory duties of the Local Authority. Experience in either Adult’s Services or a multi-disciplinary setting would be desirable, and you will need to be able to demonstrate your knowledge and understanding of the Care Act assessment process, and other relevant Adult Social Care Legislation.

Hybrid working: Enjoy the benefits of hybrid working arrangements, aligned with the needs of the service. You will be required to attend the office 3 days a week.

Key responsibilities:

1.       Conduct face-to-face assessments of service users' needs, risks, and capacities.

2.       Manage a varied caseload and prioritise competing demands

3.       Fulfil duty social work functions as per the rota, including duty visits and face-to-face interactions.

4.       Undertake Care Act assessments, Safeguarding Adults Enquiries, and Mental Capacity assessments.
