About the role: 

As a Commissioning Broker, you will be at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, you will help secure high-quality care packages and placements that truly make a difference. 

In this role, you will: 

  • Support safe and timely hospital discharges, working closely with partners to ensure everything is in place 

  • Manage a diverse and sometimes complex caseload 

  • Maintain accurate and up-to-date records across systems 

  • Handle sensitive information with professionalism and confidentiality 

  • Monitor provider performance to maintain high standards of care 

  • Work collaboratively with managers to improve processes and outcomes

About you: 

You are a motivated and organised professional who thrives in a busy environment and enjoys working with people. 

You will bring: 

  • Strong communication and relationship-building skills 

  • The ability to prioritise, plan, and make sound decisions 

  • A proactive approach to problem-solving 

  • Confidence adapting to changing priorities and demands 

  • Experience using Microsoft Office and other IT systems 

  • Excellent attention to detail, particularly in administrative tasks
