We are looking to appoint a temporary Care Facilitator to be responsible for negotiating and arranging appropriate packages of care for individual service users, as specified by Social Workers, Care Coordinators and Trusted Assessors. You will be a member of the Care Facilitator and Direct Payments Team within Adults and Health Services.

This post is temporary until 31 March 2027.
 
WHAT IS INVOLVED?
You will work closely with Social Workers, Care Coordinators, Trusted Assessors, care providers and other stakeholders to secure appropriate care and support services for adults across County Durham. The role involves coordinating care packages, managing individual procurement exercises, maintaining accurate records and systems, monitoring provider capacity and contributing to the effective delivery and ongoing development of the service. As part of a busy team, you will need excellent organisational, communication and negotiation skills, with the ability to manage competing priorities and build effective working relationships.

WHAT WILL I NEED?
Required Qualifications:

  • 5 GCSEs including English Language (9-4/A-C grades) or equivalent relevant qualification.
  • NVQ/Apprenticeship level 2 in Health and Social Care, or equivalent qualification 
  • Excellent ICT skills

Interviews will be held on 23rd and 24th September 2026.
