We have an exciting opportunity for a Social Care Practitioner to join our Mental Wellbeing Service!

We are looking for someone who has experience of delivering Statutory Adult Social Care Services, or experience working with vulnerable adults who have mental health conditions, to join our Mental Wellbeing Service. 

This is a full-time, permanent role with hybrid working, and a salary of £26,895 - £30,132 per annum.

This is an excellent opportunity for an enthusiastic Social Care Practitioner to join our Mental Wellbeing team.

The role:

You will be reporting to the Service Team Manager / AMHP Lead.  Your role is to work alongside the psychiatric inpatient liaison, carry out Care Act assessment and coordinate community resources in a timely manner to facilitate safe discharge.

Your role will involve:

  • Managing a workload and facilitating hospital discharge in a timely manner. Promoting multiagency working with health and social care colleagues.   
  • Undertaking Care Act assessments and reviewing care package and placement in line with the Care Act 2014.
  • To deliver person-centred service and adopt strength based approach with a view to promote people’s independent living in the community.   

What we are looking for:

  • Experience of working with people, preferably experience with those who are vulnerable or have disabilities.
  • Knowledge of relevant legislation.
  • Someone who understand and applies the principles of person-centered care.
  • Works within own skill level, identifies learning needs and seeks support appropriately.
  • Have good communication skills both written and oral, including report writing.
