It is an exciting time to come join us in Adult Integrated Care. Using a strengths-based approach to care, our vision is to promote independence and meeting the health and wellbeing needs of vulnerable people. We are looking to recruit enthusiastic and hardworking, post qualifying Social Worker within our Community team for learning disabilities, in the Wakefield District. You will work closely with our key partners in the CTLD Health and Intensive Support Team, to carry out person led assessments with people aged 18 and over with a learning disability. Based on strengths to promote positive change, self-determination and to prevent harm to people with an appearance of social care needs, in order for people to achieve their personal outcomes.
The team consists of Social Workers, Care Co-ordinators and Co-ordination Support Officers. As Social Worker, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am – 5pm and to 4.30pm on a Friday.
As a team player, you will need to demonstrate an ability to, work under pressure, be well organised and able to effectively prioritise and risk assess/manage a complex caseload. You will have or be ready to learn to build your skills and experience, knowledge and have a commitment to strength-based assessments and support planning. The ability to travel is essential.

All newly qualified staff will need to undertake the ASYE programme which Wakefield are dedicated to, you will be given an ASYE assessor to help you along the way with regular 1:1 sessions, training and support from the team. This first year in employment will be a probationary year as we work with you to pass and to start your career in Social Work.
Applicants for this role must be qualified to a Degree level or equivalent and be registered as a Social Worker with Social Work England, with essential post qualifying relevant training and experience.
Successful applicants will be required to attend an interview and if offered a post, to complete a Disclosure Application Form.
