Become part of the Hospital Team, where you'll play a vital role in supporting residents through a safe and well-coordinated discharge process. Your efforts will help ensure that individuals transition smoothly from hospital care back into the community, with the right support in place to maintain their wellbeing and independence.
This is a full-time, permanent role with hybrid working, and a salary of £38,192 - £42,012 per annum, plus an annual retention bonus of £3,000 subject to eligibility.

This is an excellent opportunity for an experienced Social Worker to join our Hospital and Community teams.
The Role:
As a Social Worker within these teams you will be helping to support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. Work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment.
You will be supported with your caseload by a Senior Social Worker and a Team Manager.
