We are looking to appoint a STAR Trusted Assessor.

18.5 hours per week

Your duties will include:

• Ability to carry out comprehensive assessments in hospital and community settings across the Borough of Sandwell
• Identify individual needs and personal outcomes through thorough assessment
• Develop personalised support plans aimed at meeting identified needs
• Complete appropriate risk assessments to identify potential risks
• Implement actions and strategies to minimise any risks identified

Essential requirements for the role are:  

• Minimum of 1 years’ experience working with vulnerable adults and carers
• Knowledge of assessment frameworks, including risk assessment
• Ability to travel throughout the borough as required
• Availability for 7‑day working, including some unsocial hours
• GCSE English and Maths (or equivalent qualification)
